THE BASICS

What is NDIS?

The National Disability Insurance Scheme (NDIS) is the Australian Government's program that provides funding and support to Australians with permanent and significant disability. It is run by the National Disability Insurance Agency (NDIA).

The NDIS funds individualised plans designed to help participants pursue their goals, build independence, and participate in community life. Funding is allocated across different support categories and can be used with registered providers like Evolving Care.

Each NDIS plan is unique — there is no "one size fits all." Your plan reflects your individual goals, daily support needs, and circumstances.

Who can access the NDIS?

To be eligible for the NDIS, you must be under 65 years of age, an Australian citizen or permanent resident, and have a permanent disability that significantly affects your ability to participate in everyday activities.

The Three Types of NDIS Funding

Core Supports

Funds everyday support needs — personal activities, community access, transport, consumables, and supported independent living.

Capacity Building

Funds supports to build independence and skills — including allied health (Improved Daily Living), support coordination, and employment assistance.

Capital Supports

Funds larger one-off items — assistive technology, home or vehicle modifications, and Specialist Disability Accommodation (SDA).

PLAN MANAGEMENT

How your NDIS Funding can be Managed

There are three ways NDIS plans can be managed. Evolving Care can work with you regardless of your management type.

Self-Managed

You manage your own NDIS funding and pay providers directly. You can use both registered and unregistered providers, giving maximum flexibility.

Evolving Care can work with self-managed participants

Plan-Managed

A registered plan manager handles your NDIS finances on your behalf — processing invoices from providers and keeping track of your budget.

Evolving Care can work with self-managed participants

We can manage your plan funding, process invoices, and give you provider choice — ask us about plan management at your next referral.

NDIA-Managed

The NDIA manages your funding and pays providers directly on your behalf. You must use registered providers only.

REGISTRATION GROUPS

What our Registrations mean for you

NDIS registration groups correspond to specific types of supports. Here's what each of our registrations covers and what it means for participants.

Innovative Community Participation

Supports participants to engage in innovative community activities that build skills, social connections, and independence in new or creative ways. May include new technology, community arts, sports, or emerging social programs.

Group / Centre Activities

Funds group-based programs at a centre or in the community — social activities, skills groups, recreational programs, and educational sessions delivered with peer support.

Assist-Personal Activities

Daily personal assistance including personal hygiene, dressing, grooming, meal preparation, and mobility support — provided in the participant's own home or community setting.

Participate Community

Supports to help participants access and participate in community activities, social groups, civic life, and recreational opportunities alongside other community members.

Assist Personal Activities High

High intensity personal activities for participants who require additional clinical or specialist skills during personal care — including complex bowel care, enteral feeding, ventilator support, and more.

Assist-Travel / Transport

Support with transportation to appointments, community activities, work, education, and social engagements. Evolving Care provides this as part of our community access services at no additional cost.

Assist-Life Stage, Transition

Supports to help participants navigate key life transitions — including leaving school, entering the workforce, moving out of home, or managing changes in care arrangements. Includes life coaching and planning support.

Daily Tasks / Shared Living

In-home and shared living assistance, including Supported Independent Living (SIL) support. Covers daily tasks in shared household environments — cooking, cleaning, personal care, and overnight support.

Household Tasks

Practical support with everyday household tasks including cleaning, laundry, grocery shopping, and home maintenance — enabling participants to live in a safe and comfortable environment.

Development-Life Skills

Programs and support to build daily living and life skills — cooking, budgeting, public transport, communication, and decision-making — helping participants become more independent over time.

EVOLVING CARE ADVANCED

High Intensity & Complex Care Registrations

Evolving Care Specialised holds 14 registration groups for participants with complex, high-intensity, or specialist needs — including plan management (0127), Module 2A, high-intensity personal activities, community nursing, and therapeutic supports.

Community Nursing Care

Clinical nursing supports delivered in community settings — including wound care, catheter and continence management, complex medication administration, and health monitoring by registered nurses.

Therapeutic Supports

Allied health and therapeutic interventions provided by qualified practitioners — including OT, physiotherapy, psychology, social work, exercise physiology, behaviour support, speech pathology, and more. Funded under the Capacity Building budget (Improved Daily Living).

Support Coordination

Specialist and standard support coordination services to help participants understand and implement their NDIS plans, connect with providers, coordinate services, build community connections, and work toward their goals.

Clinical nursing supports delivered in community settings — including wound care, catheter and continence management, complex medication administration, and health monitoring by registered nurses.
