Evolving Care Specialised is our dedicated entity for high-intensity, complex, and specialist care. Holding an NDIS registration with 14 registration groups — including plan management (0127), Module 2A, and high-intensity daily personal activities (0104) — it provides the full spectrum of specialist support for participants who need more. Together with Evolving Care, we deliver a complete continuum of care under one trusted team.
